If One Chooses Plan G-HD (billing)

If one were to choose Plan G-HD how is billing handled ?

I know Medicare picks up the first 80%, then Plan G kicks in, if one chooses G-HD how is billing handled ?

Example: Say Doctor visit is 200.00, Medicare Part B would pick up 160.00 and Plan G-HD would pick up the 40.00, does the Doctor bill me for the 40.00 ? Or do they send to Plan G-HD and they pay him ? Does Plan G-HD then send me a bill for the 40.00 ? Who keeps track of the amount until you reach the deducible ?
